Summary:
The Community Health Worker serves as a bridge between under-served communities and the health and social service systems. This is a non-clinical role that will work closely with our social workers, helping to address our patients' social determinants of health and connecting them with appropriate community resources. The Community Health Worker will also work with patients that have chronic disease and help them to navigate through the health system.
Essential Job Functions:
Serves as a bridge between underserved communities and health/social service systems.
Works closely with social workers to address patients' social determinants of health (e.g., food, housing, transportation).
Assists patients with chronic diseases in navigating the health system.
Advocates for patients' comprehensive health needs, including social determinants of health.
Engages patients using strong communication skills, non-judgmental perspective, and effective listening.
Builds relationships within the community and deliver presentations.
Serves as a point person for patients, helping overcome barriers while maintaining boundaries.
Documents interactions and activities as required.
