Mayo Clinic is seeking individuals interested in the newly developed Care Transitions Program. The part-time role will assist in the tracking and management of discharged patients from Mayo Clinic Hospital defined as high risk for readmission. Activities will be diverse including remote monitoring of patients at next level of care facilities, triaging calls, providing on site assessments within the hospital proper and occasionally may include home visits.
The Nurse Practitioner or Physician Assistant performs the following tasks in any authorized setting (clinics, hospitals, ambulatory surgery centers, nursing homes, or patient's home).
- Obtaining patient history
- Performing physical examinations
- Ordering and performing diagnostic and therapeutic procedures
- Formulating a diagnostic impression
- Developing and implementing a treatment plan
- Monitoring effectiveness of therapeutic interventions
- Offering counseling and education to meet patient needs
- Making appropriate referrals
- Prescribing and dispensing prescription medication
- Demonstrates ability to assess data reflective of patient's status and appropriately interprets information relative to patient's age-specific needs.
A valid driver's license is required.
Must be a graduate of CAAHEP (Council on the Accreditations of Allied Health Education Programs) approved university program. NCCPA certification required. Must be licensed/certified or eligible to practice as PA in the State of Florida.
Master's degree in Nursing required. (If licensed prior to 1998, Master's degree not required by the State of Florida.) Must have current Florida Advanced Registered Nurse Practitioner (ARNP) license or eligible to obtain Florida license. Certification by the ANCC in a specialty area required.
Post-acute care experience and General Internal Medicine experience are preferred. Two years of experience, in related clinical specialty preferred.
The candidate selected must have well developed communication and physical assessments skills, be able to work independently and excellent critical thinking decision making skills. The candidate must have excellent time management skills and effectively prioritize responsibilities.
License or certification
Physician Assistant: NCCPA certification required. Must be licensed/certified or eligible to practice as PA in the State of Florida.
Nurse Practitioner: Current ARNP license to practice in the state of Florida required. ANCC Specialty certification required.
BLS certification required.
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum salary every 2 weeks is approx $xxxx.xx, based on a full-time position.
Hours / Pay period
40 hours per pay period with some flexibility is required
Monday-Friday; 3:00-11:00 p.m.; some schedule flexibility
Occasional weekend coverage is required
