Nurse Case Manager I/II (Job Family) - Iselin, NJ
Please Note: This position may be filled at either the Nurse Case Manager I, or Nurse Case Manager II level. Manager will determine level based upon the selected applicant’s skillset relative to the qualifications listed for this position.
This is a telephonic case manager position in the NJ Health Plan with the primary responsibility of performing initial assessments on members and reviewing assessment for care management. No field work involved.
The Nurse Case Manager is responsible for care management within the scope of licensure for members with complex and/or chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum.
