The goal of the Immunization Coordinator is to help enhance access to high-quality medical services in order to improve the overall health status and lower the cost of care for the community that CDHC serves. As part of the population health team, the Immunization Coordinator will focus on quality of care and cost of care metrics associated with immunizations (childhood, adolescent, and adult). The Immunization Coordinator will help improve these metrics through use of population health tools to identify gaps in care and targets for intervention. The Immunization Coordinator will work with the Population Health Manager and other members of the population health team to review current approaches and develop data-informed improvement strategies. The Immunization Coordinator will monitor and manage vaccine inventory and storage at all CDHC locations and will monitor and track compliance with immunization programs such as Vaccines for Children (VFC) and others. The Immunization Coordinator will be instrumental for covid vaccination planning and distribution. The Immunization Coordinator will provide education related to immunizations to CDHC patients in an effort to improve access to services.
POSITION-SPECIFIC DUTIES & RESPONSIBILITIES
Assists with development of goals and objectives for population health team
Understands and is adept in using population health tools within CDHC EHR as well as from payors and grant funders
Develops data-informed interventions to improve organizational performance in quality of care and cost of care metrics related to immunizations
Coordinates with clinical care teams to promote improvement in quality of care and cost of care metrics
Collects and analyzes population health data
Identifies patients who are due for immunizations and coordinates efforts to contact them to schedule needed immunizations
Manages vaccine inventory and storage within CDHC to include ordering of vaccines, receipt of vaccines, and monitoring of vaccine storage refrigerator and freezer units to ensure integrity of vaccine stock
Provides subject matter expertise to CDHC regarding current immunization guidelines and use of the Nebraska immunization database (NESIIS)
Maintains current knowledge of regulations and reporting requirements for VFCas well as other immunization programs in which CDHC participates
Produces reports as needed or requested
Coordinates with clinical and outreach/education staff in planning and executing immunization events
Develops and conducts staff training as appropriate and necessary
Participates on internal committees related to clinical outcome measures and performance improvement
Participates actively within the Immunization Task Force (ITF)
Performs other duties as assigned
POSITION REQUIREMENTS
Education: Bachelor's degree (health-related field preferred) or Nursing degree
Experience: 1+ years in ambulatory health care
Experience in population health efforts or quality improvement preferred
Expertise: Strong interpersonal skills
Proficient verbal, written and reporting skills
Microsoft applications (experience with Excel required), internet, email
Good record keeping methods
Ability to achieve measurable outcomes
Strong organizational skills
Ability to handle multiple tasks
Knowledge of medical terminology
Language: English fluency
Work Hours: 40 hrs/wk

Exposure: While performing the essential functions of this job, the employee occasionally is exposed to fumes or airborne particles and toxic or caustic chemicals. Additionally, the employee may occasionally be exposed to blood borne and other hazardous chemicals. Finally, the noise level in the work environment can occasionally be quite loud.
