Senior Community Worker at Public Health Foundation Enterprises, In in San Mateo, California

Job Description:


The Senior Community Worker will oversee and monitor a team of Community Workers, working remotely for the San Mateo County Department of Public Health.  The Senior Community Worker works closely with the Public Health Nurse Hospital Discharge Planners, San Mateo County Health Communicable Disease Contact Tracer Team community agencies to facilitate placement of residents needing safe places to be in isolation or quarantine due to COVID-19.

This position is required to work weekends and holidays.  Normal hours are between 8:00 a.m. to 6:30 p.m.  

ESSENTIAL FUNCTIONS


  • Leads and monitors a team of Community Workers, including scheduling, review and approval of timesheets, and schedules regular 1:1 check-in with each Community Worker

  • Oversee the assignment of referrals from Contact Tracers for placement into AHS site.

  • Develop processes and/or procedures as needed for contact referent and interview to determine if meet AHS site requirements. 

  • Ensure that referrals are being placed as appropriate.

  • Monitors Community Workers’ use of the scripts, policies, and procedures provided by Public Health when conducting the interview and placement of the referral.

  • Ensure complete documentation in Electronic Health Record by Community Workers.

  • Documents the number of interviews and placements each Community Worker conducts per day/week and takes corrective actions of any issues regarding uneven distribution of assignments, or unfilled referrals.

  • Collaborate with a Public Health Nurses (PHN) and Contact Tracers (CT) to ensure safe isolation and quarantine for COVID-19 affected residents.

  • Attend daily team huddles.

  • Conduct weekly 1:1 check ins with the community workers

  • Coordinate with HSA and BHRS regarding any AHS guest issues that arise.

  • Disseminates new materials to the interview team as updates are made

  • Complies with all required Public Health trainings

JOB QUALIFICATIONS


  • Experience in supervising teams.

  • High degree of technology skills, required, including use of computers, software, and platforms.

  • Sound critical thinking and judgement skills.

  • Ability to interact professionally with culturally diverse individuals during a time of crisis and distress.

  • Knowledge and application of HIPAA laws.

  • Collaborative skills required working across multiple teams and disciplines.

  • Ability to problem solve various situations related to COVID positive and COVID negative patients in upholding local public health and CDC guidelines.         

Education/Experience


  • Bachelor’s Degree, preferred.

  • One to two years of experience in a leadership/supervisory role, managing a team, addressing, and resolving problems, preferred.

Remote

Must have functioning WIFI in your remote location to work remotely.  There is no reimbursement for the WIFI.
