Nurse Practitioner with 3 years experience as an NP and 3 years experience managing a patient with Diabetes.

Certified Diabetes Educator preferred

The Diabetes clinic is a Multidisciplinary clinic comprised of NP, RN-CDE, Dieticians, Medical Assistant.

The Nurse will manage Type I, Type II Diabetics as well as patients with Gestational Diabetes.

The NP will be rounding on inpatient floors to meet the management needs of the Diabetes patients. Participate in patient education and discharge planning.

Referrals to appropriate follow-up care.

Education to staff re: Diabetes management.

Secondarily:

The Nurse Practitioner will participate in one on one appointments as well as patient Education Classes. Triaging care by phone is also part of this role.

The Nurse Practitioner will be practicing independently under the Supervision of our Medical Director who is and Endocrinologist.

Regular review of practice with the Supervising Physician will be incorporated into the work schedule.

Participation in outcome measures consistent with our ADA program.

Prescriptions and maintaining refills for this patient population.

The NP will work closely with the referring physicians.

Competence with documentation in an electronic medical record.

Primarily inpatient responsibilities, but also includes outpatient orientation and coverage.

Qualifications

Current MA License

Graduation from an Accredited DNP program

3 years practicing as a Nurse Practioner

3 years participating in the management of Diabetes Patients

Certified Diabetes Educator preferred

Current DEA and NPI nos.
