Description:

The PACE Referral and Appointment Coordinator is responsible for managing the work queue and scheduling external and internal appointments in collaboration with participant/caregiver, taking participant preferences into account. The Coordinator will schedule all transportation related to appointments. The Coordinator will manage the referral work queue for external appointments, scheduling those appointments at the appropriate location, and entering the appointments in EPIC; The Referral and Appointment Coordinator will also collaborate with the secretaries from the PACE sites to ensure good coordination between PACE Center, IDT, Participant/Caregiver and external vendors.

Education

  • HS Diploma/GED required.

Experience

  • Certification in transportation software support preferred, or completion of certification within 6 months of hire.

Skills/Abilities


  • Strong computer skills: Windows, Outlook, and experience with EMR preferred.

  • Strong customer service skills, in person and over the phone

  • Bilingual Spanish speaking skills required
