Hybrid Schedule-Hybrid is defined as a flexible work model that supports a blend of in the physical office (1-2 days per week) and partially remote workplace.

JOB PURPOSE 

Care Coordination Associate assist BCDSS permanency staff with coordinating health care services for children in out of home placement, including scheduling of appointments for routine medical, dental, mental health and specialty care as needed. 

ESSENTIAL DUTIES 

The primary responsibilities of this position are: 


  • Serve as a resource person for BCDSS permanency staff for navigating the medical, dental, and mental health systems. 


  • Track health care services for children in out of home placement to assure their health needs are being met including retrieving medical documentation for medical case review 


  • Identify health care resources and make referrals to appropriate agencies. 


  • Teach and role model appropriate health-seeking behaviors to DSS staff, foster and kinship families 


  • Participate in staff, community, professional and inter-agency meetings and conferences as necessary 

Additional duties and responsibilities: 


  • Document assessments, activities, treatment plans,  and resolutions in database. 


  • Update medical information and MATCH database 


  • Update medical information in CHESSIE 


  • Answer the telephones and assist callers 

QUALIFICATIONS: 

Education, Experience, Certifications and Licensures: 


  • Bachelor’s degree in social work or health related field 

KNOWLEDGE, SKILLS & ABILITIES 


  • Experience in coordinating care/services for clients 


  • Well-developed interpersonal & communication skills 


  • Proficient in MS Word, Excel, , Outlook and Gmail 


  • Professional appearance and demeanor 


  • Familiarity with database systems 


  • Customer service skills 


  • Ability to effectively gather information via telephone in a professional manner 


  • Ability to document appointment scheduling outcomes 


  • Ability to answer a multi-line telephone system and direct calls to the appropriate person 


  • Familiarity with community based resources 


  • Ability to gather community-based resources that are useful for the populations served by the MATCH Program 


  • Excellent oral and written interpersonal and communication skills required 


  • Problem solving skills 


  • Proven ability to work efficiently and meet deadlines
