MTM is looking for a Community Health Worker

This role is a mix of remote (work from home) and fieldwork, with visits to the patients home as required based on their needs assessment. Candidates must reside in the State of Nebraska (Rural areas of the State preferred)

Pay: Up to $30 per hour based on experience

What Will Your Job Look Like?

The Community Health Worker (Mobile Integrated Health) is focused on making an impact in the communities we serve, by conducting member outreach, coordinating care and assisting with identifying and referring to local resources with a goal of improving patient outcomes in conjunction with MTM partner health plans.

This role is community facing and will liaise frequently with both patients and health plans to identify and provide the necessary resources to improve the health and well-being of our members, including health needs assessments, appointment coordination, scheduling transport, housing/ food/ behavioral health needs, and follow-up. 

The Community Health Worker will own the delivery of a high level of service to our members and vendors every day, ensuring high operational standards and member satisfaction within an assigned geographic area.

What You’ll Do:


  • Establish outreach to members and conduct home visits

  • Proactively identify and implement ways to improve the health and well-being of assigned members

  • Manage the day-to-day interaction with the vendors, acting as an Account Manager to build and sustain strong working relationships with cross functional departments and assigned Providers

  • Serve as the first point of contact for all vendor service requests and escalations

  • Demonstrate proactive leadership by working with internal MTM teams and external client teams, when necessary, to ensure complete resolution of issues

  • Recognize opportunities to educate partners on MTM processes when necessary

  • Schedule medical referrals, diagnostic tests and treatments following outlined protocols, assure patients are notified, understand instructions, and complete all required documentation

  • Ensure medications and supplies needed for patient care received and educate on instructions for use

  • Develop program specific patient education

  • Handle inbound service issues, troubleshoot problems, concerns, and complaints

  • Develop, implement, review and revise policies and procedures

  • Accountable for achieving monthly, quarterly, and annual patient outcome and satisfaction goals

  • Work closely with MIH Director and other team members to develop/ maintain/ grow relationships with local community-based organizations (CBOs) and EMS providers and providers with a goal of increasing services available for MTM members

  • Assist in developing, and adhere to referral process for providers and CBOs to refer members for necessary services

  • Answer incoming calls from assigned members and provide them with information, materials, and end-to-end assistance with a wide range of services to assist with improving individual health and wellness

  • Build positive relationships with the community, including assisting with local networking and marketing activities

  • Gain knowledge of and understand all aspects of the Client and the contract

What You’ll need:


  • High School diploma or GED equivalent

  • Work experience as an EMT or Paramedic experience strongly preferred

  • Bachelor’s Degree or equivalent, job-related experience preferred

  • Minimum 2 years’ experience working in the medical field and knowledge of community health, case management and/ or care coordination

  • Experience with customer service

  • Experience with home health care, case management, emergency medical services, community paramedicine, or mobile integrated health strongly preferred

  • Experience working with Medicaid and/or State programs

  • Experience with large healthcare programs aimed to reduce hospital utilization, cost of care initiatives, improving HEDIS measures and gap closures a plus

  • Bilingual a plus

  • Must possess a valid driver’s licenses and insurance

  • Moderate skills in Microsoft Office

  • Must be willing and able to attend required training and comfortable meeting patients in their homes

  • Must be easily adaptable to ever changing environment and requirements

  • Ability to solve practical problems dealing with a variety of variables

  • Demonstratable personal traits of a high-level commitment, motivation; team orientation; professionalism; trust; personal integrity; and a demonstration of placing others in a place of high value and treating them with dignity and respect

  • Uphold confidentiality and HIPAA compliance at all times and report violations to Privacy Officer

  • Knowledge of Federal, State and local regulations pertaining to care at home or mobile integrated health a plus

  • Knowledge of managed care, quality improvement, and contracting a plu

  • Willing and able to attend and successfully complete any relevant training or certifications as required by MTM and the Client
