IPS Employment Case Manager

Full-Time
Tues. - Sat. 8am - 5pm

We are currently seeking positive, upbeat, outgoing candidates for the position of IPS Case Manager. As an employment case manager, you will make a difference each day, helping individuals with Severe and Persistent Mental Illness find employment. You will help building relationships with employers to open doors that wouldn’t otherwise be opened to our targeted population.

GENERAL RESPONSIBILITIES AND ESSENTIAL JOB FUNCTIONS


  • Provide Case Management (CPST, Crisis) to persons assigned to caseload.

  • Develop an individual employment plan within 14 days of referral.

  • Conducts on and off-site job coaching as indicated.

  • Provide Benefit Counseling (Social Security Benefits and Medicaid)

  • Makes 3-5 contacts per week for job development.

  • Maintains ongoing relationships with employers and potential employers.

  • Completes all necessary paperwork directly related to the position and care of persons in services within the designated time frame.

  • Meet with referral within 72 hours of referral.

  • Facilitate groups related to employment.

  • Weekly contact sheets turned in at the end of the workweek.

  • Progress notes are completed within 24 hours of service.

  • EMR scheduler is utilized and kept updated.

MINIMUM QUALIFICATIONS:


  • Age 21+ years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience.

  • Excellent interpersonal and teamwork skills

  • Excellent organizational and time management skills

  • Interest and investment in working with adults who experience Severe and Persistent Mental Illness (SPMI) and their families.

  • Have completed or will complete any required training within six months of employment.

  • Must have a current and valid Kansas driver’s license.
