SCOPE OF ROLE


Reporting to the Director, Infectious Disease Services, the Specialist, Disease Intervention will work in collaboration with service providers to effectively coordinate medical, prevention and other support services for patients recently diagnosed or at a risk of contracting HCV and/or other infectious diseases while assuring access to and retention in care. The Specialist, Disease Intervention will identify individuals who use drugs and are exposed to STI’s through high-risk behaviors. The Specialist, Disease Intervention will provide education, and other risk reduction information about HCV prevention and care as well as care prevention for other Infectious Diseases and assure they are treated appropriately and follow ups completed.


 


HOW YOU’LL SUCCEED:



  • Conducts time -limited linkage to care interventions to coordinate medical, prevention, and other support services for patients recently diagnosed or at a risk of contracting HCV and/or other infectious diseases while assuring access to and retention in care. Provides education, and other risk reduction information about HCV prevention and care as well as care prevention for other Infectious Diseases and assure they are treated appropriately and follow ups completed.

  • Develops and extracts reports from databases for delivery to internal and external candidates. Enters infectious case information into the Ohio Disease Reporting System (ODRS) Attends team meetings and support calls as assigned.

  • Establishes and maintains effective working relationships with host area officials, agencies and individuals who have impact on the infectious disease program in order to enhance relationships, ensure effective control of HCV Infectious Disease in the community and to make strategic contributions to program planning, policy development, and procedural implementation.

  • Prepares summaries and completes required paperwork, ensures the submission of required paperwork from assigned staff.

  • Administers HCV project protocols and workflow.

  • Serves as liaison between health center and local public health department.

  • Serve as a key health center participant in HCV DIS ODH project.

  • Work to support infection control and Covid activities of the health center.

  • Assist with linkage to treatment for identified infectious diseases.

  • Interview cases, contacts and others in the community concerning personal behaviors and activities which contribute to disease transmission; make collateral contacts to obtain pertinent information concerning other individuals who may have been exposed or infected; ensure compliance with established State and federal laws and regulations related to disease control and prevention.

  • Act as patient advocate; counsel clients regarding medical and social services available and intervene on their behalf for access to services or resolution of conflict.

  • Counsel and educate patients before testing, patients receiving test results and post test results.

  • Other duties as assigned.


KNOWLEDGE & EXPERIENCE:



  • High School Diploma or equivalent required. Bachelor’s Degree in related social science field preferred.

  • 2+ years’ experience in public health, social work, health education, community service or medical setting preferred.

  • Basic knowledge of local, state and federal guidelines related to HCV, HIV and Syphilis transmission and prevention preferred.

  • American Heart Association (AHA) Basic Life Support (BLS) certification required.

  • Strong computer skills with Microsoft, Excel, etc.

  • Knowledge of the community, community organizations, and community resources.


WORKING CONDITIONS:



  • Work is normally performed in a typical interior/office/clinical work environment.

  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.

  • Requires periods of sitting, standing, telephone, and computer work. 

  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.

  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.

  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.

  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.

  • Possible exposure to blood borne pathogens while performing job duties.

  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.

  • Sufficient dexterity to operate a PC and other office equipment.
