Job Description:

Duties and Responsibilities:
Develops and implements agency wound care program to include patient care protocols, wound care formulary and WOCN referral criteria.Performs home visits and/or telephone consults as per WOCN referral criteria, patient and staff need.Makes recommendations to the physician for changes to wound care orders and provides evidence-based research support as needed.Provides evaluation visits to all patients who are being treated for daily or twice daily wounds by the Agency; evaluates treatments and visit frequency.Follows up with nurses regarding effectiveness of recommended treatmentsProvides consultative services for ostomy and incontinence skin care patients; making home visits as needed; instructing nurses in procedure, equipment and management of such patients.Attends care management meetings to participate in team conferences and offer insight into patient care, options or alternative treatments to promote healing, and promote effective and efficient nursing care.Provides basic wound care and ostomy care training and orientation to all new nursing staff; provides ongoing in-services and training related to wound care, ostomies and incontinence.Through training and support, empowers all clinical staff to effectively assess wounds and recommend appropriate protocols.Works with vendor(s) to maintain cost-effective wound care formulary.Utilizes digital photography of wounds for consults and outcome documentation. Makes recommendation to Director Clinical Operations.Maintains professional certification: keeps current in areas of practice.Adheres to Nursing Care Standards, Agency Policies/Procedures, State, Federal, and CHAP guidelinesParticipates in Performance improvement activities, utilization review, and PACEmployee adheres to all applicable federal, state, local laws and regulations.Other duties as assigned.
