We are looking for a dynamic MDS Nurse (RN) who is a leader with supervisory experience and who is passionate about the team's success and easily builds and maintains positive relationships with peers. The ideal candidate will have a strong background in LTC Skilled Nursing and has demonstrated leadership abilities. This position requires excellent communication skills, attention to detail, and the ability to work well under pressure.

Successful candidates will have the following:


  • MDS Experience in a Long-Term Care Setting (Required)

  • Nursing Degree from an accredited college/university


  • ACTIVE, unencumbered RN license to practice in South Carolina (Required)

  • PDPM experience required

  • Ability to conduct and document accurate assessments of patients

  • Ability to interview patients and families to ensure an accurate picture of the patient's psychosocial/medical status is obtained

  • Ability to coordinate with other disciplines to ensure accurate and timely completion of the MDS.

  • Organizational skills, ability to schedule and meet deadlines

  • Monitoring the overall medical record to validate support of MDS coding

  • Works with interdisciplinary team to assure timely completion of Resident assessment Protocols and Patient Care Plans

  • Supports the accuracy of medical record documentation through ongoing in-services to center partners.

The primary function of the MDS Nurse is to assess resident care needs, direct and supervise staff to meet the resident's needs, coordinate with other members of the Inter-Disciplinary Team (IDT) develop and implement a plan of care that meets the individual needs of each resident. The MDS Coordinator nurse also provides direct resident/patient care.
