Make a bigger difference

Overview:

The Case Worker works directly with the entire familyreferred to the preventive services program by providing a range of services,both directly and through referral that ensure safety and stability for thechildren.

RESPONSIBILITIESinclude but not limited to:

  • Provide a range of concrete and clinical services andintervention to ameliorate the reasons family became known to the preventiveservices program.
  • During the intake and assessment phase, introduce FamilyTeam Conferencing (FTC) model to the family.
  • Using both ACS and JBFCS tolls, assesses and reassess theindividual members of the family in areas of safety and risk, mental health,physical health, substance abuse, domestic violence, housing and employmentneeds.
  • Locate and ensure appropriate community resources forclient; communicate with community providers ongoing and request theirparticipation in FTC.
  • Develop service plans in concert with the family as wellas safety interventions.
  • Provide clinically focused counseling to all family members.Identify and provide access to specific appropriate concrete services, whennecessary or requested, and are in the best interest of the client.
  • Maintain frequent face to face contact, meeting at leastminimum standards of home based and office visits including observing all ofthe children monthly.
  • May run groups for parent or for youth.
  • Maintain the case records consistent with JBFCS and ACSstandards.
  • Other duties as assigned.

REQUIREMENTS:

LMSW preferred, MSW or BA Degree with some relevantexperience in child welfare or family counseling services.
