Position Overview: Under the direction of the Community Engagement's Director and Vice President, the Health Equity and Diversity Liaison will facilitate diversity, equity and inclusion work internally and externally to address the root causes of health inequities. This will involve building capacity to integrate community insights into efforts to develop system level changes that advance health equity in the Coordinated Care Organization (CCO), its governance structure and provider networks. The liaison will work locally within the CCO region to reach out to and engage community-based organizations representing populations that experience health disparities; to advocate for and ensure meaningful and effective relationships so these groups are not only represented but also actively engaged in collaborative decision making with our Community Advisory Councils and other CCO governance entities.
Advance the national Culturally and Linguistically Appropriate Services Standards (CLAS) to better serve a diverse membership
Support implementation of the Oregon Health Authority's health equity training requirements
Identify local resources to advance CLAS; support development and dissemination of CLAS tool-kits for external partners
Implement community-level transformation activities that address the social determinants of health and equity (SDOH-E).
Develop and maintain positive relationships with a wide variety of community members and entities (e.g. organizations representing diverse populations, schools, law enforcement, and social service agencies) that have an interest or need to improve health equity within the communities served by the CCO.
Participate on other local CCO and community boards or work groups to bring an "equity lens" to community health strategies and initiatives that impact PacificSource Medicaid members, including informing Community Health Assessment and Community Health Improvement planning processes.
Convene and facilitate community groups to gather insights that inform system changes that advance equity, including ensuring meaningful community input into the development, implementation and monitoring of the CCO's Health Equity Plan.
Support development and implementation of SDOH-E Transformation and Quality Strategy activities and reporting, with engagement of health councils and community partners.
Work Experience: Three years of experience in healthcare, public health or community based organization required. Experience working to advance diversity, equity and inclusion efforts in healthcare or community systems. Experience preferred in community networking and/or facilitating groups.
Education, Certificates, Licenses: High school diploma. Associates or Bachelor's degree preferred Cultural responsivity training experience preferred.
Knowledge: Knowledge and understanding of social determinants of health and health equity and a commitment to diversity and inclusion program implementation to reach desired outcomes. Relationship building and community collaboration. Experience giving presentations. Experience with writing reports preferred. Organizational skills with solid experience in using computers and various software applications. Ability to work independently with minimal supervision. Bi-lingual strongly preferred.
